Error: No Records Found


If you have no record found in the VaxCertPH, the details of your vaccination record may have been incorrectly encoded or have not been uploaded yet in the central database by your vaccination center. You may coordinate with your LGU or the vaccination center where your vaccines were administered to upload your details in the central database where the VaxCertPH website gets the data. You may also request for an update of your record through the website.

The Philippine vaccine certificate, VaxCertPH, has been recognized in 30 countries including the United States, Canada and the United Kingdom, the Department of Foreign Affairs said.

Just the same, vaccine certificates issued by these 30 countries will also be recognized by the Bureau of Immigration upon arrival in the Philippines. Starting February 16, only fully vaccinated foreigners are allowed to enter the Philippines.

Only foreigners from these 30 countries may present their national/state digital certificate as part of the reciprocity agreement with the Philippines.

If foreigners were inoculated from countries which has not recognized the VaxCertPH, they should present WHO-issued International Certificate of Vaccination or Prophylaxis (ICV) as proof of Covid-19 vaccination.

But if the foreigners are spouses, children, parents of OFWs, and they are traveling with the OFWs to the Philippines, the national digital/physical vaccination certificate where they were vaccinated may be presented even if the host country they come from did not recognize the VaxCertPH. Alternatively, they may also opt to get a certificate from the Philippine Overseas Labor Office.

VaxCertPH App is a web portal that issues digital vaccination certificates for fully vaccinated individuals in the Philippines. 


Fully vaccinated individuals can begin applying for unified digital vaccine certificates issued by the national government by September.


The absence of a unified vaccination card has started to cause hurdles for OFWs who need to travel abroad, the most recent of which is Hong Kong’s refusal to accept vaccination cards issued by local government units.


Recently, the Hong Kong government is refusing to recognize the vaccination certificates of individuals fully inoculated against COVID-19 in the Philippines. The government of Hong Kong won't honor the proof of vaccination issued by local government units and the Bureau of Quarantine because it is not "connected to a single source."


This obstacle delays the departure of over 3,000 Filipino workers who were scheduled to fly back to their employers in Hong Kong starting Monday. The Department of Foreign Affairs said the yellow cards would serve as a temporary solution to the problem.


HongKong is open to accepting the World Health Organization-recognized International Vaccine Certificate or "yellow card" issued to fully vaccinated OFWs. However, the Bureau of Quarantine needs to include the traveler's passport number.

The Philippine national vaccine certificate system had its soft launch for overseas Filipino workers (OFWs), as well as international travelers from Metro Manila and Baguio City last September 6, 2021.


The digital vaccine certificate, VaxCertPH, will be opened to the general public soon according to Presidential Spokesperson Harry Roque.


“Bibigyang prayoridad sa unang phase ng pagpapatupad ng VaxCertPH ang [OFWs] at mga Pilipinong aalis pa-ibang bansa kung saan ang place of residence ay Metro Manila at Baguio City. Bubuksan ito sa general public at iba pang mga bagay at a later time,” Roque said.



Here’s how to get a vaccination certificate from VaxCertPH


The Philippines' online portal for COVID-19 vaccination certificates, VaxCertPH, is now online and accepting registrations.

1. To register, visit vaxcert.doh.gov.ph and fill up the forms. 


2. You will be first asked to read and agree to the data privacy statement, which indicates that the information shared will be used for your VaxCertPH application.


3. After ticking the box, the site will ask you if you are an international traveler or an OFW in Metro Manila who will travel outside the country in the next two weeks.

4. If you answer "no," you won't be able to continue with your registration as the portal is still in its soft launch stages. It will prioritize travelers within the National Capital Region (NCR) and Baguio.


5. Clicking "yes," there'll be a prompt asking for your current vaccination status and when you received your final dose.


6. On the next page, you will input your personal information and your vaccination details. Make sure that the details you put match the ones on your vaccination card.

7. Then you will be asked to review the information you provided. The form will also ask you to type in a code for verification purposes.

8. The site will look up your vaccination records and if all the information is correct, it will ask you to prepare a photo or scanned copy of your vaccination card and a government ID.


9. After submitting the necessary documents, the portal will ask for your email and contact number.

10. Then, you will get a reference number, which means your request was successful. All you have to do is wait for an email or message for the digital vaccination certificate.

Those who are encountering difficulties with the online portal may coordinate with their LGUs to register at VaxCertPH booths.

VaxCertPH to be implemented by October says DICT

The Department of Information and Communications Technology (DICT) Undersecretary, Emmanuel Ciantic, confirms the full implementation of the VaxCertPH by October of this year.

VaxCertPH relies on the data submitted by local government units (LGUs) through the Vaccine Information Management System (VIMS). DICT urges all LGUs to submit their respective Line Lists of vaccinated individuals through the DICT Vaccine Administration System (D-VAS) to aid the rollout of the upcoming VaxCertPH.


VaxCertPH would be fully implemented to both overseas Filipino workers and workers in Metro Manila; with other areas soon to follow.


DICT will have a dry-run on NCR and OFWs in the meantime to perfect the overall process.

The Information and Communications Technology department (DICT) will launch the VaxCertPH before September ends and will be implemented by October.


The rollout of VaxCertPH will begin in Metro Manila and for overseas Filipino workers (OFWs) for free.


The VaxCertPH is different from the Quarantine Bureau’s (BOQ) Yellow Card, which is mainly for travel.


"Siyempre bago nagka-COVID is Yellow Card. So 196 country members, yellow card 'yan. Yellow fever, measles, and whatever. Ang kumplikado po kasi sa COVID-19, mas kailangan niyan ng datos na may first dose, second dose, anong brand, kailan tinurok,” DICT Undersecretary Manny Caintic says.


Vaccinated persons will just have to register on the portal the DICT will soon launch.


For the meantime, the BOQ has also opened express lanes online for OFWs who wish to apply for a Yellow Card.

Overseas Filipino workers (OFWs) will be using government-issued “yellow cards” while the government works on a unified vaccine card.  The Philippine government was in the meantime implementing Bureau of Quarantine-issued yellow cards (ICV), while working on a uniform digital vaccination card equipped with a quick response (QR) code.

VaxCertPH, a digital certificate, is given to individuals who have been fully inoculated against the virus. It consists of the person's name, the date and location where the jab was administered, and the brand of the vaccine.

The Department of Information and Communications Technology (DICT) aims to launch in September its VaxCertPH or the digital vaccination certificate for individuals fully inoculated against Covid-19. The system for VaxCertPH was ready and has been integrated with the World Health Organization countries, but they want to finish training the local government units (LGU) first before the launch. LGUs were on their third week of training in preparation for any questions or guidance that the public may have or need once the system was in place.

The DICT clarified that the VaxCertPH is different from the Bureau of Quarantine's Yellow Card, which is also honored in different countries. 


Vaccinated individuals can apply for a VaxCertPH via the DICT's online portal. The certificate bears the individual's name, the date and location where they received their jab, and the name of the vaccine.


DoH will handle the data, and the DICT will help protect it. The issuance of vaccine certificates will be private key encrypted, and the Health department remains to be the lone issuer of the certificates

THE Inter-Agency Task Force for the Management of Emerging Infectious Diseases (IATF-EID) on Friday moved for the creation of a sub-technical working group (TWG) that will focus on the issuance of vaccination certificates as part of government measures during the Covid-19 pandemic.


"The IATF approved the constitution of a [sub-TWG] to design, deploy, support and recommend relevant policies related to the national certificate for Covid-19 vaccinations, otherwise known as the 'VaxCertPH,'" Palace spokesman Harry Roque Jr. said.

DILG hinikayat ang LGUs na i-upload ang tamang datos sa Vaccine Information Management System (VIMS) sa loob ng 24 oras


Matapos ilunsad ang VaxCertPH program sa NCR ngayong buwan, hinikayat ng Department of the Interior and Local Government (DILG) ang mga lokal na pamahalaan na madaliin ang pag-upload ng kumpleto at tamang detalye sa Vaccine Information Management System (VIMS) ng DICT upang makakuha ng digital vaccination certificates ang mga fully vaccinated na mamamayan.


“Ang VaxCertPH ay kailangan para ma-isyuhan ng digital vaccination certificates ang mga kumpleto na sa COVID-19 doses sa bansa. Kaya dapat siguruhin ng mga LGUs na maipasok ang tamang datos sa VMIS sa loob ng 24 oras mula ng mabakunahan,” sabi ni DILG Secretary Eduardo M. Año.


Sinabi pa nito na ang digital certificates na iiisyu sa ilalim ng VaxCertPH ay magiging patunay sa mga Filipino at non-Filipinos na nagpabakuna sa Pilipinas na may balak maglakbay palabas ng bansa o umuwi galing sa ibang bansa, bilang pagtugon sa requirement ng ibang bansa na magpresenta ng vaccine certification mula sa Philippine Department of Health ang mga nais pumasok sa kanilang teritoryo.


Kasalukuyang nasa soft-launch, ang VaxCertPH ay unang magagamit ng mga OFWs at mga dayuhang maglalakbay sa Metro Manila. Ito ay ilulunsad din sa buong bansa pagkatapos ng pilot period sa NCR.


May 20.9 million Filipino na ang fully vaccinated ng Covid-19 vaccine kung saan 27.1% ang kabilang sa eligible population. Sa NCR, 5.1 million na ang fully vaccinated, kung saan 52.37% ang kasama sa eligible population, habang 8,261,461 (85.50%) ang nakatanggap na ng unang dose.


Binuo ng Department of Information and Communications Technology (DICT) para sa Department of Health (DOH), ang VaxCertPH ay “self-service web portal” na ginagamit ang vaccination data na isinusumite ng LGUs sa pamamagitan ng VIMS.


Mayroon itong request at creation feature ng COVID-19 Digital Vaccination Certificate; gumagawa ng QR (Quick Response) code kung saan authenticated ang certificate; mayroon din itong request feature kung saan ang kliyente ay puwedeng mag-request ng updating ng records kung may maling vaccination details o missing information.


Ayon kay Año, ang VaxCertPH ay sumusunod sa mga guidelines na inisyu ng World Health Organization (WHO) para sa Digital Documentation ng COVID-19 Certificate (DDC).


Sinabi pa nito na dapat kilalalinin ng mga LGUs at ipatupad ang VaxCertPH bilang official proof of vaccination para sa mga lokal na biyahero at magtalaga ng LGU coordinator para sagutin ang mga katanungan at ayusin ang mga record-related issues ng vaccinations.


Sa DILG Memorandum Circular (MC) No. 2021-095, inaatasan ang mga local chief executives (LCEs), DILG Regional Directors (RDs), at Minister for the Local Government ng Bangsamoro Autonomous Region ng Muslim Mindanao (BARMM) na aktibong lumahok at suportahan ang pagsasanay na isinagagawa ng DICT tungkol sa VaxCertPH.


Sa ilalim ng Republic Act 11525 o COVID-19 Vaccination Program Act of 2021, inatasan ang DOH na mag-isyu ng vaccination cards sa lahat ng bakunado. Ang DICT, sa pakikipagkoordinasyon sa DILG at DOH, ay inatasan ding bumuo ng digital process na magsesertipika na ang isang tao ay binakunahan sa bansa.
