Q: What is an International Certificate of Vaccination or ICV?

A: The International Certificate of Vaccination or ICV is a document that records an individual’s vaccinations required to travel abroad. It is recognized by member countries of the World Health Organization.

Q: Is ICV mandatory for those going abroad?

A: ICV is not mandatory and cannot be considered an “immunity passport.” It serves as “proof of vaccination” of an individual. Not all countries require ICV. You should contact the country you are going to to find out what their entry requirements are.

Q: Who may request for ICV for COVID-19?

A: Individuals who will travel abroad, such as Overseas Filipino Workers (OFWs), seafarers, and others, can obtain an ICV for COVID-19. These individuals must be “fully vaccinated” in the Philippines.

Q: What are the needed requirements in requesting for an ICV for COVID-19?

A: Passport, valid ID, and vaccination card are required. The following must be uploaded to the system and must be presented on the day of your appointment.

Q: How can I apply for an ICV for COVID-19?

A: You need to book an appointment at www.icv.boq.ph

Q: Is there a charge for an ICV for COVID-19?

A: There is a fee for requesting an ICV for COVID-19. It costs Php 300.00 + Php 70.00 convenience fee

Q: I was vaccinated with the first dose abroad, and my second dose was in the Philippines. Can I get an ICV for COVID-19?

A: Currently, only those who were “fully vaccinated” in the Philippines can be issued an ICV

A: Currently, only those who are “fully vaccinated” in the Philippines can be issued an ICV

Q: I made a booking mistake. Can I get a refund?

A: The terms and agreement state that entering incorrect information may result in your payment being forfeited.

Q: I failed to go to my schedule. What will happen to my schedule and payment?

A: It is stated in the “terms and agreement” that not attending your scheduled appointment may be a reason to forfeit your payment. However, if the reason for not going is medical, you can email helpdesk.clinic@quarantine.doh.gov.ph and attach the medical certificate proving your condition.

Q: I cannot attend my schedule. Can I send a representative with an authorization letter to process the service I booked?

A: Senior citizens, PWDs, and those with medical conditions that may be a reason for failing to attend their appointment can delegate an “authorized representative” to collect their ICV. However, one “authorized person” can only take up to two transactions. They must bring an authorization letter for each transaction and must present two valid IDs. They must also present a document proving the client’s condition, such as senior citizen ID, medical certificate, and others.

Q: My ICV does not have a passport number. Do I need to get a new ICV?

A: Starting August 20, 2021, all ICVs issued by the BOQ will have a passport number.

However, all ICVs issued before that date shall remain valid and will not need to be replaced. If the ID you uploaded when booking an appointment is your passport, it has already been automatically updated to digital form. There is no need to issue your ICV. If you do not have a passport, you can email helpdesk.clinic@quarantine.doh.gov.ph and request that your passport number be updated on the ICV. Send your reference number, copy of ICV information page and your passport information page.

Q: I am going to Hong Kong, and my ICV does not have a passport number. What should I do?

A: You can request certification from the BOQ Office. The certification and ICV must be presented to the authorities upon arrival in Hong Kong.

1. Pa-deploy na ako. Paano mapapabilis ang pagproseso ng International Certificate of Vaccination (ICV) ko sa Bureau of Quarantine (BOQ)?

Magbook lamang ng schedule sa https://icv.boq.ph. Pagkatapos nito ay maaaring magwalk-in ng mas maagang petsa sa Bureau of Quarantine. 

Kailangan ring dalhin ang mga sumusunod: 

(a) valid ID; 
(b) passport; 
(c) vaccination card, at; 
(d) proof of deployment. 

Maaaring employment contract ito na may deployment date at flight details. Kung wala pang kontrata, maaaring magrequest ng sulat sa inyong manning agency na nagsasabing madedeploy ka na sa nakasaad na petsa.


2. Bakit may bayad na P370 ang pag-issue ng ICV?

Ayon sa BOQ, ang P300 ay bayad ng standard documentation fee, at ang P70 naman ay convenience fee sa paggamit ng online platform. Ang yellow card na ito ay parte ng international convention na napagsang-ayunan ng mahigit 200 na bansa. May format na sinusundan ang BOQ, base sa agreement nila sa iba pang naturang bansa, kaya may bayad ito.


3. Paano kumuha ng ICV kung nagpabakuna ako sa ibang bansa?

Kapag fully vaccinated ka sa ibang bansa (first dose at second dose, kasama na rin ang single-dose Janssen), doon ka rin sa bansa na yon makakakuha ng ICV. Pero kung nasa Pilipinas ka na, mas mainam nang magpabakuna ulit para makakuha ng ICV sa BOQ.

Kung nakapag-first dose ka naman sa ibang bansa at second dose sa Pilipinas, maaaring kumuha ng ICV sa BOQ. Dalhin lamang ang proof of vaccination sa dalawang lugar, kasama ang ibang requirements na nasa #1.

Vaccination Certification for the Outbound Travelers from the Philippines


The public is hereby informed that the implementation of the interim guidelines issued by the National COVID-19 Vaccination Operations Center (NVOC) applicable to outbound travelers from the Philippines until the Certificate of Vaccination Record Portal becomes fully operational.


The following are important points to follow:


1. Outbound travelers who have been fully vaccinated in the Philippines may apply for an International Certificate of Vaccination (ICV) or "Yellow Card" with the Bureau of Quarantine (BOQ) if such is required in the country of destination.


2. They may register for appointment through https://boq.pisopay.com.ph/  which will now redirect you to https://icv.boq.ph/  


When making an appointment through the BOQ Medical Services OBS platform, applicants for ICV for COVID-19 must be able to pay through electronic means the amount of Three Hundred Pesos (PhP 300.00).

Please be informed on guidelines shared by BOQ on the recently opened satellite office at MOA for Issuance of the International Certificate of Vaccination (Vaccine Passport).

1.  Booking can be done through the following websites

3.  For now, the BOQ will be prioritizing the travelers- those with overseas contracts or booking confirmations.  BOQ will open this to others when they are able to increase their capacity with acquisition of additional machines.

4.  Slots for MOA for schedules July 19  Monday onwards will still open after lunch today.

BOQ warns ‘yellow card’ applicants on submitting fake documents


August 16 - Individuals who are planning to get a yellow card or International Certificate of Vaccination and Prophylaxis (ICVP) will face penalties if they submit fake documents, the Bureau of Quarantine (BOQ) warned. “Mahigpit tayo. Meron tayong mga pinapatupad na ways para masigurado na hindi talagang makapag submit ng peke na dokumento yung mga gustong kumuha ng yellow card (We are strict. We have implemented ways to ensure that those who want to get a yellow card cannot submit fake documents),” said BOQ Director Dr. Roberto Salvador in an interview over radio DZBB on Sunday, August 15.


“Pinapaalala natin kapag nag forge kayo, nag submit ng peke, pwede po kayong mademanda at ma-revoke po ang passport niyo o matanggal sa trabaho (We are reminding the public that if they forge or submit a fake document, they can be sued, their passport could be revoked, or they could be fired from their jobs),” he added. The yellow card provides details of the vaccination history of an individual. This can be presented by individuals—who need to travel abroad—to their country of destination.


On Saturday, the BOQ appealed to the public to refrain from applying for a “yellow card” if they do not have a scheduled trip abroad—to ensure that those individuals who have an immediate schedule to travel overseas will have a slot in getting the said document.
