STEP-BY-STEP GUIDE


2. Click I AGREE to the Privacy statement
3. Click REGISTER NOW!
3.1. Click INTERNATIONAL
4. Input all the necessary details
  - make sure that you have input your ACTIVE EMAIL ADDRESS
  - make sure that you input your CORRECT SRN NUMBER (Can be found sa MISMO Acct nyo)
5. Check I HAVE READ AND HEREBY AGREE TO THE TERMS OF USAGE OF THIS PORTAL
6. Click SIGN UP
7. You will be redirected to your dashboard - click UPDATE INFO
8. Update ALL YOUR DETAILS. Make sure that you input/update all your details.
  - Please make it sure that you input your correct MANNING AGENCY so your records will be tagged to your Manning Account
9. Click SAVE afterwards.
10. Click UPDATE VACCINATION RECORD
11.1 If already vaccinated in your own LGU, Click YES and update and upload your vaccination cards
11.2 If NOT YET VACCINATED, click NO. Select Vaccination Site: AMOSUP MANILA
11.3 Priority Group: Please input A1. Subpriority can be left blank. Pls put NO to Allergies and Commorbidities.
12 Click SUBMIT - you will be redirected to APPOINTMENT PAGE Afterwards.
13. Under Select Type: Select AM or PM then click SUBMIT
14. After clicking submit, AVAILABLE APPOINTMENT DATES WILL APPEAR. Please select your desired appointment date.
15. Click SET APPOINTMENT
16. You will be redirected to the AMOSUP PORTAL afterwards
17. Register your account in AMOSUP PORTAL  https://amosup.upnextware.com/
   It will generate and give you a QR Code that you will need to bring during your vaccination day.
   All you vax record and vaccination card will be recorded in your QR Code.
18. Go back to MARINA COVAX PORTAL, click MY PROFILE, and Click CONFIRM APPOINTMENT
19. Your appointment will be secured by that time.

1. LAS PIÑAS VACCINATION

Vaccination Program will continue on Aug. 2 to 4 at the Tent Villar Sipag Global Complex C5, Las Piñas City.

Open to all Seafarers - walk-in / onsite registration is allowed. Please advise your Seafarers / Cadets to bring their SIRB/SID for verification.

2. MANILA VACCINATION

With the announcement of ECQ from AUg 6-20 in Metro Manila, the scheduled vaccination in Amosup Manila on August 2-6, 2021 will now be August 2-5, 2021 (MONDAY to THURSDAY)

 

An amended date/time slot will be sent tonight.

 

Kindly provide ORIENTATION to all your seafarers to follow all instructions indicated below to avoid any delays on the day of their vaccination.

1.       The Manning Agency is responsible to inform the seafarer about his/her vaccination date/time slot and requirements.

 

2.       Seafarers who have received any kind of vaccination (Polio, Hepa etc.) within the last 14 days will not be allowed for Covid-19 vaccination.

 

3.       Seafarers must arrive at the venue at least 15 minutes before his/her time slot.  Do not arrive too early nor too late to avoid congestion at the site.

 

4.       Seafarer must bring the following to avoid delays:

ü  Seaman’s book or Seafarer’s ID. Photocopy is acceptable.

ü  Ballpen

ü  Hand hygiene kit (for his personal use)

ü  Umbrella

ü  Accomplished and printed Vaccination forms (please see attachments of below required form):

a.       Health Declaration Screening Form for Moderna

b.       Informed Consent Form for the Covid-19 Vaccine Moderna

c.       Covid19 Vaccination Card (Health Facility: Amosup Sen’in No Ie Building)

 5.       The date for the 2nd Dose Moderna vaccination is scheduled after 28 days from the 1st dose which we will reconfirm to the LMAs.

INSTRUCTIONS TO MANNING AGENCIES:

Dear Manning Agencies,

 

MARINA gets 30,000 doses of Moderna Covid19 vaccine exclusively for seafarers which will be distributed within week 30 in major vaccination sites throughout the country.  Of this number, 15,000 are allocated for the 1st dose and another 15,000 for the 2nd dose.

 

To register your seafarers, please follow the instructions below:

 

1.      Fill out the attached Excel file.  The Excel file contains several sheets corresponding specific locations with specific additional instructionsLMAs are responsible to fill out the attached Excel file; NOT the seafarers.  The Excel file contains several sheets that correspond to specific locations with additional instructions per vaccination site.  The use of the Excel Template prepared by the Marina TWG is strictly required.

 

2.      Register your manning agency and upload the Excel file thru the link below:

https://forms.gle/6hNaZqbs14eVv7fq7• IMPORTANT: only one (1) file submission is required per LMA.  In other words, all confirmed seafarers of your agency must be registered in one Excel file at one time.

• TWG will select the most recent Excel file submitted by the LMA who have submitted multiple files.

 

3.      Deadline of submission is on Tuesday, 27 July, 2021 at 1200hrs. For Las Pinas, today, Monday 26th July at 1700H.  The Marina Technical Working Group will evaluate the volume of submissions and will reply with the final confirmed list, date and time slot by the end of day.    Please do not send seafarers at the site if they are not confirmed.

 

4.      Seafarers who have received any kind of vaccination (Polio, Hepa etc.) within the last 14 days will not be allowed for Covid-19 vaccination.

 

 

On the day of vaccination, please remind your seafarers to:

5.      Arrive at the venue at least 15 minutes before his time slot.  Do not arrive too early nor too late to avoid congestion at the site. 

 

6.      Bring the following to avoid any delays:

·        Seaman’s book or Seafarer’s ID. Photocopy is acceptable.

·        Ballpen

·        Hand hygiene kit (for his personal use)

·        Printed registration form with QR code (Applicable only to all Amosup locations and Bataan)

 

7.      Wear facemask and face shield and observe physical distancing at all times.

 

8.      Take maintenance medications, if any, before going to the site.

 

9.      Get enough sleep day before the scheduled vaccination

 

The date for the 2nd Dose is scheduled after 28 days.  Exact schedule will be indicated in the vaccination card, and time slot will be the same as the 1st dose. 5. Should there be a need for LMA to revise the list due to urgent reasons i.e. replacement for crew who tested positive of covid-19 or other valid reasons, please email us here: marinatwgvaccination@gmail.com

  1. Why is it that seafarer will register first to LMA? what if he/she has no LMA yet? However he/she wanted to be vaccinated? If seafarer registered already in MARINA like if he/she has already SRN number, COE/COC, MARINA SID, is these information not enough for your verification? why is it that necessary to go LMA first? Why seafarer can't register directly to MARINA COVID19 vaccination online without passing through LMA?
